Making the Claim
The D&R application consists of three main parts:
- Member Statement
- Physician’s Statement, and
- Employer Statement.
The initial 119 calendar days after you become Totally Disabled are known as the Elimination Period. To make a D&R claim, all three parts of the application must be fully completed and submitted to HEB Manitoba within 60 days after the end of the Elimination Period (within 180 days from the Date of Disability).
The sooner your completed claim application is received, the sooner HEB Manitoba can assess your eligibility for D&R Benefits.

Once all original, completed claim application forms are received by HEB Manitoba, your claim will be assessed. If there is additional information that is pertinent to your claim, please provide supporting documents with your application.
Notes:
- Under the terms of the D&R Plan, a late application will be denied unless there is a written explanation satisfactory to HEB Manitoba, as to why the application could not be submitted within 60 days after the end of the Elimination Period (within 180 days from the Date of Disability). If it is not possible for you to submit your application within 60 days after the end of the Elimination Period, you can still submit your application with supporting documents. However, you must also provide a letter explaining why your application is late.
- Original completed, signed, and dated forms must be returned to HEB Manitoba. Photocopies will not be accepted. Exception: HEB Manitoba will accept photocopies of Proof of Age documents.
HEB Manitoba recommends that you complete and submit your application as soon as possible to avoid delays in assessing your claim.
For a Recurrent claim, you must reapply for D&R Benefits (the three parts of the application form will need to be completed and submitted to HEB Manitoba). For example, if you were approved for a D&R claim, returned to work to your full duties and normal level of hours, and then within six months of your return, again become Totally Disabled from the same or related cause, your claim may be considered under the Recurrent Total Disability provision of the Disability and Rehabilitation Plan Text.
